Please contact me by phone or email to schedule your first appointment.

I will then send you a link to the confidential client portal, for you to complete intake paperwork prior to our first appointment. You may fill out the client intake and upload it to the client portal, or mail a copy to the office prior to our first appointment. Please bring a copy of your insurance card to the first appointment.
